Case Manager/Counselor Tester
OVERVIEW OF POSITION
This is a full-time position. Half of the time this position is for Case Management which will have the primary responsibility of managing all services that link with health care and psycho/social services in a manner that ensures timely, coordinated access to appropriate levels of care to persons living with HIV/AIDS. The other half time the position is for a Counselor/Tester which delivers pre and post counseling to individuals seeking HIV/AIDS testing. The procedures for testing and counseling are established by the Massachusetts Department of Public Health.
RESPONSIBILITIES
- Will provide information and referrals services to approximate caseload of 42 clients.
- Conduct an assessment of client’s needs and personal support system.
- Develop a comprehensive individualized service care plan for each client.
- Conduct periodic reevaluation and adoption of the plan.
- Submit monthly and quarterly reports on all activities to Program Director.
- Facilitate Pre & Post Test counseling sessions with individuals and groups. Conduct risk assessment.
- Conduct oral and/or intravenous blood testing for the detection of HIV antibodies.
- Identify and maintain working relationships with potential testing sites.
- Conduct outreach activities to reach Latino immigrants and hard to reach communities such as, sex industry workers, drug users, and men that have sex with men. Conduct street outreach as a team to maintain consistent testing rate to meet required MDPH contract agreement.
- Serve as Spokesperson and Advocate of Centro Latino to the community on HIV/AIDS issues.
- Actively participate in community events and health fairs.
- Attend all required meetings as assigned.
- Other task as assigned by the supervisor.
REQUIREMENTS
- Ample knowledge of HIV/AIDS and related issues.
- Demonstrate interpersonal, communication, writing, and presentation skills.
- Understanding of risky behaviors, substance abuse, sexuality and relevant psychosocial issues.
- Certified in HIV counseling and testing strongly preferred or able to obtain certification within three months of employment.
- Experience in drawing blood preferred.
- Two to five years of experience in HIV/AIDS field and must be fluent in English and Spanish.
- Must have strong, computer and writing skills. Excellent interpersonal and communication skills.
Volunteer Mentors for Middle School Youth
Centro Latino de Chelsea is looking for Volunteer Adult Mentors for its afterschool program – Jóvenes Latinos Saliendo Adelante (JLSA – Young Latinos Moving Forward). This program helps 12-14 year old youth in Chelsea to develop the academic skills and character competencies to prepare for succeed in high school and beyond. As a mentor, you will help children to build relationships, develop confidence, and achieve their goals.
Requirements and Qualifications:
- Must be 18 years or older.
- Willing to work with either boys or girls, one-on-one or with up to four children at a time.
- Able to commit to 2 hours per week for the entire 8 month school year.
- Able to work well with a culturally diverse community.
- Able to attend two-hour training.
- Must agree to a CORI and SORI background check.
- Most of all, mentors must have a strong desire to work with youth and support them academically and socially.
The Mentors at Jóvenes Latinos Saliendo Adelante Program:
- Understand that the relationship is the key to mentor and mentee success.
- Take responsibility for building a relationship with their mentee.
- Follow through on commitment.
- Respect the youth’s viewpoint.
- Support the youth in solving their problems.
- Provide feedback on a regular basis to the program coordinator.
- Rely on the program for support.
English as a Second Language Instructors (Part Time)
Centro Latino has an ongoing need for part-time ESL instructors for our English Communication at Work classes. Centro needs to identify qualified teachers for future projects that are being developed as well as substitute teachers that are available on an on-call basis. Generally, each class will meet for three hour sessions, twice a week for up to 20 weeks per cycle. Classes generally run during the morning or late afternoon and evening, and some on Saturdays. Total teaching hours average 6-12 paid hours per week (+ paid prep time).
Minimum requirements: Bachelor’s Degree in Education or related field and demonstrated classroom experience working with adult learners. Must have strong interpersonal communication and language skills. Mass. Teacher and/or ABE licensure a plus. Compensation: $23.17 per hour.
Spanish Instructors (Part Time)
